COMMUNITY POSTPARTUM PROGRAM

 

APPLICATION FOR OUR POSTPARTUM PROGRAM

We are excited to announce that we have started the new Community Postpartum Program.  This program is designed for the following families:

  • Unattached mother-baby dyads

  • Families who struggle getting to their postpartum appointments due to transportation or childcare barriers, and

  • Families who could benefit from extra support, breastfeeding counselling, and home visits (ie. twins, etc).

The Community Postpartum Program is covered by BC Medical Services Plan, and is an extension of regular midwifery postpartum support for clients that were not in our care for pregnancy and birth.  Our catchment area is Peachland to Lake Country, and we are currently accepting referrals from any healthcare practitioners caring for mother-baby dyads, including obstetricians, pediatricians, family practitioners, nurse practitioners, and public health nurses.

We also accept self referrals from families wanting additional support. If you are interested in becoming a postpartum client, please fill out the form below. Please be aware that we often have wait lists and space is limited. We take a limited number of clients each month and encourage families to apply for care early to ensure they get a spot.

The goal of the Community Postpartum Program is to provide high quality, community-based postpartum support through regular home visits in the first weeks of new parenthood for families looking for a little extra support. Visits include well-woman and newborn care along with close breastfeeding support from Registered Midwives. Our program is covered by your BC CareCard and will work in conjunction with your delivering provider.
